The professionals at Care Management Advocates do not replace family. We augment the concern you already feel. However, because you face time constraints, distance, unfamiliarity with available resources, or conflicting needs, you find yourself unable to do what is in your heart to do for your loved one. Consequently, we manage the care on your behalf. 

Which of these is applicable to your situation will be determined as a result of the development of the individualized Plan of Care. Because we are well connected with many other entities, we will be able to specify who does what, how often, to what degree, and so on. 

Depending on the specific situation, Care Management Advocates' professionals may fill some of these roles themselves.
