Care Management
When it comes to caring for yourself, a family member or a loved one, navigating the elder or disability care network can be daunting. Very few people have the time and/or knowledge necessary to coordinate care for themselves or an aging or disabled love one. Care Management Advocates has experienced staff that is available 24/7 to coordinate and manage care for you or your loved one. After a comprehensive assessment of your or your loved one’s needs, we will develop a long-term care plan that tailors services and care that is uniquely designed to foster safety, happiness and well-being. We then monitor the on-going care and provide services such as transportation to and from dental and medical appointments; shopping and social outings; and scheduling home maintenance and repairs. Of course, all of our care planning takes into consideration the needs and desires of you or your loved one, as well as your financial situation. Coordinated and managed care often can actually reduce overall care costs because our staff networks with a variety of caregivers and ensures that no overlap occurs. We remain totally independent when selecting vendors by accepting no referral fees.
Guardianship
Colorado law provides for protection of “incapacitated” persons by establishing guardianships. A guardian has authority to make decisions regarding the person’s support, care, education, health, and welfare. This is a responsibility that we at Care Management Advocates take seriously. Whether we become guardians because there is no family available to step into the role, or whether the family can’t decide upon the care and turns to a professional guardian, as a member of the National Guardianship Association we follow the NGA Ethical Principles in treating the person with dignity and involving the person to the greatest extent possible in all decision making. Primarily, we provide the same assessment, care planning, monitoring and service providing that we offer our care management clients, while at the same time ensuring compliance with the statutory requirements for guardianship.
Guardianship Support Services
In situations where a family decides that a family member or another person familiar with the family is best suited to serve as a loved one’s guardian, we can assist, especially when the appointed guardian lives out of state. We can take as much of the burden off the guardian as needed by offering whatever care management services the family wants or needs. Most importantly, we will assist in completing and filing the annual guardian report.
Medical Power of Attorney
You may not need care management now, but should the need arise in the future, it is best to be prepared. By serving as your primary or secondary medical power of attorney, we get to know you and your needs, wishes and desires before the need arises for assistance with medical care decision making. We will develop a relationship to continually know you and your needs.
